The three paths to group medical
For employers under 50, the real wins come from alternative-funded coverage. The right path depends on your group's health, size, and how much you want to hand off.
1 · PEO Co-Employment — the comprehensive, name-brand option
Your employees join a Professional Employer Organization's master large-group plan — with real national-brand networks (names like Aetna, UnitedHealthcare, Kaiser Permanente, Cigna, and Blue Cross Blue Shield) plus HR, payroll, benefits administration and ACA compliance bundled in. Because the PEO's entire book carries the risk, your group isn't underwritten — even a small or less-healthy group can land large-company pricing and acceptance.
Applied Business Solutions
Co-employment through a national PEO master plan.
Best for: A 2–50 employer that wants true comprehensive, nationwide name-brand coverage, wants to offload HR / payroll / compliance, and either can't pass underwriting or simply wants rate stability — big-company benefits without running benefits.
2 · Level-Funded / Self-Funded — the lowest premium for a healthy group
You pay a fixed monthly amount (claims fund + admin + stop-loss) on real PPO networks, and a healthy group can be refunded unused claims surplus at year-end. It's lightly underwritten — a short group health questionnaire — so the healthier the group, the lower the rate.
Allstate Level-Funded Group
Group major-medical on a nationwide network.
OneHealth
A broad-access community health plan — not a narrow network.
Presidio
Fair-price, any-doctor-nationwide coverage.
Best for: a relatively healthy 5–50 group that wants the lowest real-network premium and likes surplus-refund upside.
3 · ERISA Guaranteed-Issue — accept-anyone, fast
ERISA-governed plans that enroll members with no individual medical underwriting — guaranteed acceptance, no health questions — usually on a national PPO (MultiPlan / PHCS).
LifeX / XGroup
ERISA health plans, with an employer level-funded layer.
Population Science ("America’s Choice")
A MultiPlan PPO with a BCBS network option.
Enrollment First
Guaranteed-issue health, dental, accident & term life.
Best for: groups or owners that can't pass underwriting, heavy 1099 / self-employed rosters, subsidy-ineligible clients, or anyone who wants fast, no-health-questions enrollment with a real PPO.
Need to satisfy the ACA employer mandate?
For applicable large employers, we can layer affordable Minimum Essential Coverage (MEC) plans — such as Apex and USA+ — to satisfy the mandate's "Part A" penalty cheaply. These are a layer underneath a real plan, never a group's only coverage.
Group supplemental & voluntary — often the most-used part of the benefit
This is where we really specialize: supplemental options that employers and employees both love — and that people actually use the most. Layer them onto any plan above, or offer them on their own.
